If you move please let us know so that we can update your records. If you change address locally, it is your responsibility  to send your medical card to the Primary Care Support service at 187 Ewell Road, Surbiton, Surrey KT6 6AU.

If you move out of the practice area it will be necessary for you to find another doctor. The receptionist can give you more information.
