If you are collecting prescriptions or results of any kind on behalf of another patient this is known as a Subject Access Request.
As of 29th October 2015 due to national guidelines this practice can no longer issue this information without written permission from the given patient. Consent must be provided in writing via note, letter or email. Once consent has been given a note will be made for our records for future reference.
The Practice retains the right to refuse supply of a request should written consent not be provided.
