Antenatal Care - Patient Self Referral

Please complete the online questionnaire. At the end of the questionnaire there is a tab to submit the form direct to the hospital. The referral is then processed by the hospital and you will be invited for an appointment.

It is recommended that you self-refer prior to nine weeks, although this is not always possible, and can be done even if past nine weeks.
